This form is meant to provide scholars an opportunity to reach out to Program Counselors with concerns they may have regarding meeting their requirements.
This form will be e-mailed to a Fiat Lux Program Counselor and the scholar will receive a general e-mail confirmation of the submission. While the e-mail confirmation will not display it, the Program Counselor will still have access to all the information that has been provided.
If a scholar has not heard back from a Program Counselor via e-mail within a week of submitting a form, the scholar is encouraged to e-mail the Fiat Lux account at fiatlux@ucmerced.edu.
This form will close on Monday, November 22nd, 2021 at 8:00AM so students have time to complete accommodations or suggestions made by a counselor.
As a reminder requirements are due Tuesday, November 30th at 11:59 PM.
