2022-2023 Student Staff Information

Hello Student Staff, please fill out this webform by July 1st, 2022 at noon so we can gather information in regards to your housing situation, contact information, your name tag preference, and shirt size. 

Women's style is on the left and Men's style is on the right. Please note that the shirt design may change depending on what Image Masters has available.

This is the name (first & last or first/middle/last names) that will be used for your professional name tag. It does not have to be your full legal name; however, make sure it is appropriate (no nicknames).
Work polos will be in an adult size made out of polyester material (does not shrink). They will be button-up for males and v-neck for females.
