Collaborative Community Scholar Meeting Notes 2019 - 2020

 

These notes will reflect on the discussion that you had with your Community Scholar. Their purpose is to ensure that you are given the correct resources to aid in your success here at UC Merced. They also serve as a receipt of your meeting so that you are given credit towards your program expectations. 

Please identify which number of your meeting requirements this is. Ex: Is this your first time meeting with a Community Scholar?
Please ask the student if they foresee any difficulty in completing program requirements. If they do, please have them e-mail a Program Counselor or the Fiat Lux E-mail account.
Community Scholar will list main topics that student would like to talk about.
If applicable, this is the place to list the next steps that the student plans to take for each topic that was addressed. For example, "I will go to my Professor/TA office hours next Tuesday for Math 5" and "I will get 7-8 hours of sleep this upcoming week by going to bed by 10 pm"
Community Scholar will indicate any additional observations that were not initiated during the check-in. If the need arises, professional staff can utilize this information.
Community Scholar will follow-up through email with their supervisor if a Student of Concern Form was submitted.
