This form is to be completed within 24 hours of meeting with a Career Consultant, Career Counselor, Career Specialist or other designated representative from CCPA. If you fail to complete the form within 24 hours of this meeting, you will NOT receive Fiat Lux Requirement Credit.
If you do not provide accurate information, the Fiat Lux Scholars Program will NOT be able to provide you with Fiat Lux Scholar Program Requirement Credit.
