Meeting with Academic Advisor Submission Form

Meeting with an Academic Advisor Submission Form is to be completed within 24 hours of meeting with an Academic Advisor or Representative. If this form is NOT completed within 24 hours, you will NOT recieve credit for the Fiat Lux Scholar Program Requirement. 

If you do not provide accurate information, the Fiat Lux Scholars Program will NOT be able to provide you with Fiat Lux Requirement Credit.
