Workshop Requirement Form

The Workshop Requirement Form is to be completed when you attend workshops that are hosted by another UC Merced department and NOT by the Fiat Lux Scholars Program.  

This form needs to be completed  within 24 hours of attending a Fiat Lux Approved Workshop. If you fail to complete the form within 24 hours of attending the workshop, you will NOT receive Fiat Lux Workshop Requirement Credit.  

Note: You can fulfill your workshop requirement by attending  Fiat Lux Scholars Program workshops and DO NOT need to submit this form.  

If you do not provide accurate information, the Fiat Lux Scholars Program will not be able to provide you with Fiat Lux workshop requirement credit. 

Please provide us with the name of at least one of the presenters and/or the UC Merced department that hosted this workshop.
