Fiat Lux Counselor Meeting Webform '18 - '19

This webform is to be completed with the student and a Fiat Lux Scholars Program Counselor. It is meant to provide documentation of the counselor meeting for both the student and the Professional Staff member.

Please be sure to complete a webform submission during each Fiat Lux Scholars Program Counselor meeting. 

This may include challenges that the student is currently facing.
Counselor will list the next steps that the student plans to take for each topic. If this is the 2nd or 3rd meeting, this can be the space to follow-up with goals that were previously stated.
Counselor will ask students about how their requirements are going. If needed, accommodations may be noted here.
Counselor will list main concerns, if any, of what was discussed during the meeting. If the need arises, professional staff can utilize this information.
