The following form should be submitted by Fiat Lux scholars who have been informed that this is their final semester in the Fiat Lux Scholars Program and have an opportunity to submit a Program Appeal to be considered for ending in good program standing with the Fiat Lux Scholars Program, receiving a Program Completion Stole, and being able to attend the event honoring those scholars who have completed their time with the program. 

Please note: Program appeal statements will be reviewed by Fiat Lux Scholars Program Professional staff and may be reviewed by the Office of Financial Aid and Scholarships Professional staff (if applicable).

Important reminder: Please be advised that Fiat Lux Scholars Program Professional Staff have various reporting responsibilities through UC Policies, which include, but are not limited to Responsible Employees (https://ophd.ucmerced.edu/responsible-employees/overview), Mandated Reporters (https://protectminors.ucmerced.edu/), and Campus Security Authorities (https://clery.ucmerced.edu/CSA). Please keep in mind that if any information submitted through this form falls within categories of required campus reporting, program staff will submit the appropriate reports and/or referrals. If any information submitted by the scholar presents a general concern, a Dean of Students Office "Student of Concern Form" may also be submitted by a program professional staff member as well.

The deadline to submit this form can be found within the email you received from the Fiat Lux account (fiatlux@ucmerced.edu). No additional documentation is needed for this program appeal, simply complete and submit this form by the designated deadline.
