This form is meant to provide current Fiat Lux scholars an opportunity to reach out to Professional Staff with concerns they may have regarding meeting their requirements in the Fiat Lux Scholars Program. Additionally, this will help us understand your current academic status with the program, whether you're on educational leave, studying abroad, graduating early, or engaged in any other academic activity (i.e. UCDC, UC Sacramento, etc.). Your responses will enable us to provide tailored support and keep our records up to date. Please fill in the details below.
This form will be e-mailed to Professional Staff in the Fiat Lux Scholars Program and the scholar will receive a general e-mail confirmation of the submission. While the e-mail confirmation will not display it, the Professional Staff will still have access to all of the information that has been provided by the scholar. If a scholar has not heard back from a Fiat Lux Professional Staff member via e-mail within a week of submitting the form, the scholar is encouraged to e-mail the Fiat Lux account at fiatlux@ucmerced.edu.
Important reminder: Please be advised that Fiat Lux Scholars Program Professional Staff have various reporting responsibilities through UC Policies, which include, but are not limited to Responsible Employees (https://ophd.ucmerced.edu/responsible-employees/overview), Mandated Reporters (https://protectminors.ucmerced.edu/), and Campus Security Authorities (https://clery.ucmerced.edu/CSA). Please keep in mind that if any information submitted through this form falls within categories of required campus reporting, program staff will submit the appropriate reports and/or referrals and the scholar may be reached out to by campus staff members connected to these reporting offices. If any information submitted by the scholar presents a general concern, a Dean of Students Office "Student of Concern Form" may also be submitted by a program professional staff member as well. Scholars may be reached out to by a staff member within the Dean of Student Office as well.
This form will open on Monday, January 26th , 2026, and this form will close on Friday, April 17th 3:00 pm. The form will close so program staff have enough time to review submitted information and follow-up with scholars before the end of the program requirement deadline.
As a reminder, all program requirements for Fall 2025 are due on Thursday, April 23rd, 2026 at 11:59 pm PST for current scholars. After this semester, the submissions results should be kept for at least the designated amount of time allocated for this kind of program record at the university.
