This form is to be completed within 24 hours of meeting with a Professor (or TA). If you fail to complete this form within 24 hours of this meeting, you will NOT receive Fiat Lux Scholars Program Requirement Credit.
If you do not provide accurate information, the Fiat Lux Scholars Program will not be able to provide you with Fiat Lux Scholar Program Requirement Credit.
