Fiat Lux Counselor Meeting Webform 19'- 20'

This webform is to be completed with the student and a Fiat Lux Scholars Program Counselor. It is meant to provide documentation of the counselor meeting for both the student and the Professional Staff member. 

Please be sure to complete a webform submission during each Fiat Lux Scholars Program Counselor Meeting. 

Please input the student's UC Merced e-mail address. The student will be sent a copy of the form via e-mail.

If applicable, this is the place to list the next steps that the student plans to take for each topic that was addressed. For example, "I will go to my Professor/TA office hours next Tuesday for Math 5" and "I will get 7-8 hours of sleep this upcoming week by going to bed by 10 pm"

Counselor will ask students about how their requirements are going. If needed, accommodations may be noted here.
This section is for the counselor and will not be e-mailed to the student in their meeting receipt. However, the Program Counselor is more than welcome to let students view what is being written in this section. Counselor can list any notes or concerns that they may have had during the meeting. If the need arises, professional staff can utilize this information at a later time. Sometimes, this can be helpful for the purpose of making sure accurate notes are recorded for a particular situation that may have occurred that the student shares.
