These notes will reflect on the discussion that you had with your Academic Scholar. Their purpose is to ensure that you are given the correct resources to aid in your success here at UC Merced. They also serve as a receipt of your meeting so that you are given credit towards your program expectations. 

Please note, a 30-minute Effective Planning Session counts for 1 study session hour.

Academic scholars will follow-up through email with their supervisor if a Student of Concern Form was submitted.
