Hello, this webform is for scholars who have been approved to do more than 1 Outside Workshop to complete their Fiat Lux Scholars Program Workshop requirements. This should only be filled out for that situation. The use of this form is granted approval by either a Fiat Lux Professional Staff member or a Fiat Lux Student Coordinator. This form can be viewed by both all Professional Staff and Student Coordinators.
A receipt will be sent to the scholar's provided UC Merced email account for confirmation of submission. Please be aware that not all that you submit will be included on the email receipt. However, if there are any concerns with the submitted form, the scholar will be contacted via email by the Fiat Lux Program Counselor or via an email from the Fiat Lux email account (fiatlux@ucmerced.edu).
Once reviewed for completion, and as long as there are no concerns with the submission, the point for this Outside Workshop will be attriubted to the individual scholar's "Fiat Lux Workshop #1", "Fiat Lux Workshop #2", or "Fiat Lux Workshop #3" (if applicable) section. If after a week from submission of the form you do not either see the point added to CatCourses or have not received a follow-up email regarding your submission, please be sure to email the Fiat Lux account (fiatlux@ucmerced.edu).
