Please note: Appeal statements will be reviewed by Fiat Lux and Financial Aid staff (if applicable).
Be advised that Fiat Lux Scholars Program Staff are both Responsible Employees and
Mandated Reporters. Keep in mind if any information is shared within submissions of this
form within those categories of required reporting, we will submit the appropriate reports
as needed
The deadline to submit this appeal form will be Monday, June 30th at 10:30 am. No additional documentation is needed for this appeal, simply fill out and submit the form.
