The following form should be submitted by scholars who have been informed that they are subject to program dismissal and would like to be considered for reinstatement into the Fiat Lux Scholars Program.
Please note: Appeal statements will be reviewed by Fiat Lux and Financial Aid staff.
The deadline to submit this appeal form will be Thursday, September 30th at 8:00 am. No additional documentation is needed for this appeal, simply fill out and submit the form.
